Inspection No. 70-1100/87-03 This refers to the routine safety inspection conducted by Mr. J. Roth of this office on June 1-5, 1987, for the purpose of reviewing your licensed program.

The inspection consisted of selected examinations of procedures and representative records, interviews with personnel, and observations by the inspector.

Three violations of NRC requirements were identified. Please reply to these in accordance with the enclosed Appendix A. That reply is not subject to clearance by the Office of Management and Budget under the Paperwork Reduction Act of 1980, PL 96-511.

In addition, we are also concerned that you did not complete corrective actions with regard to an internally identified violation. That violation

. involved the placement of unmeasured uranium bearing waste water into not-safe-by-geometry containers following a release of contaminated water from a defective centrifuge in the Building 17 Pellet Shop. It is our understanding that upon identification of the violation, the contaminated water was placed into safe geometry containers and the pellet shop supervisor was retrained in actions to be taken in similar future events. However, permanent corrective action, such as the preparation of action plans or procedures was not taken to assure that this type of violation should not recur. In your response to this letter, you are requested to provide us with information concerning the permanent corrective actions you intend to take regarding this type of violation.
