I Report No. SERI-0619-00001 Page 1 of 2 l
l ABSTRACT This report summarizes the inspections performed at the Grand Gulf Nuclear Station on the feedwater nozzles. The Ten-Year Inservice Inspection Plan (Specification MP&L-M489.1) specifies that augmented inspections on the feedwater nozzle blend radii and safe ends be performed in accordance with NUREG-0619. MP&L elected to perform the volumetric exams, required by Table 2 of NUREG-0619, during the First Refueling Outage (RF01) on all six feedwate" nozzles. This coordinates well with other planned vessel inspection activities.
The volumetric exams will be repeated every other outage thereafter.
The volumetric exams were performed by General Electric and the surface exams were performed by V. S. Testing under contract to MP&L to perform selected inservice
' inspections at Grand Gulf. The data reports for the inspections are available for review at the site.
In an effort to provide the NRC staff with the requested information, this report is being presented in the following format.
Each item listed in Paragraph-4.4.3.1(2) of NUREG-0619 is addressed separately.
a.
Startup/ Shutdown Cycles 1)
Total number of cycles at Grand Gulf:
56, total includes 21 scrams, 35 heatups and their respective cooldowns.
2)
Total number of cycles since the previous volumetric exams on the blend radii: 54, total includes 21 scrams, 33 heatups and their 4
respective cooldowns.
3)
Total number of cycles since the previous surface exams on the blend radii:
56, total includes 21 scrams, 35 heaturs and their respective cooldowns.
4)
Total number of cycles since the previous volumetric exams on the nozzle-to-safe-end welds: 51, total includes 21 scrams, 30 heatups and their respective cooldowns.
5)
Total number of cycles since the previous surface exams on the nozzle-to-safe-end welds (nozzles NSA and N4B only):
51, total includes 21 scrams, 30 heatups and their respective cooldowns.
l 6)
Total number of cycles since the previous surface exams on the nozzle-to-safe-end welds (nozzles N4C, N40, N4E and N4F only): 53, total includes 21 scrams, 32 heatups and their respective cooldowns.
b.
Summary of Methods Used and Results of Previous Inspections Volumetric (UT) and surface (PT) exams were performed on the feedwater nozzle blend radii and the nozzle-to-safe-end welds during preservice examinations. These preservice examinations resulted in three recordable indications from the surface exams and thirty-one recordable indications from the volumetric exams.

indications in the bore region of the feedwater nnzzle at 30 vessel azimuth (N4A). Two areas on this nozzle had indications exceeding the acceptance standards of ASME Section III. These areas were blend ground in approximately.10" increments until reduced to within the acceptable limits for Section III. After final grinding, the areas were re-examined by liquid penetrant and found to have no recordable indications (i.e., no indications with major dimensions over 1/16").
During the First Refueling Outage (RF01), volumetric exams were performed on the nozzle blend radii and on the nozzle-to-safe-end welds. Surface exams were performed on the outside surface on two nozzle-to-safe-end welds. The RF01 examinations resulted in no recordable indications from the surface exams and only non-relevant or I.D. surface geometry recorded during the volumetric exams.
k Tables 1 and 2 (attached) provide examination methods-and dates for each nozzle.
c.
System Changes and Changes in Operating Procedures The volumetric examinations performed during RF01 were the first examinations performed since the preservice examinations. There have not been any system changes or changes to operating procedures that would effect system tempera-ture or flow during this time period.
d.
Inspection Results During RF01, remote volumetric examinations were performed on all six feed-water nozzle blend radii and bore regions as well as the six feedwater nozzle safe ends.
No indications were recorded which required evaluation in accor-dance with ASME Section XI on the blend radii. Non-relevant indications, I.D.
surface geometry, geometry from thermal sleeve positioning ridge, and root geometry were recorded on the nozzle-to-safe-end welds.
Final analysis of the ultrasonic data in accordance with the General Electric procedure for reviewing and analyzing recordable indications (NDE-1) allowed the welds to be accepted for service.
e.
Leakage Monitoring Grand Gulf has not installed any type of on-line leakage monitoring system capable of detecting leakage through degraded seals on the feedwater spargers.
f.
Information Regarding UT Crack-like Indications and any Subsequent PT Indications No UT crack-like indications were detected, thus no penetrant testing was j
required.
