Dear Ms. McLaughlin:

This is in regard to a request for approval of a route to be used for transport of spent reactor fuel as contained in your letter of October 20, 1988.

The route from Portsmouth Marine Terminal, Portsmouth, Virginia, to the Department of Energy Idaho National Engineering Laboratory, Idaho Falls, Idaho, including the transient ports of Port Elizabeth, New Jersey and Baltimore, Maryland, as requested in your letter is judged to meet the regulatory requirements in accordance with 10 CFR 73.37 for shipment of spent fuel under import license ISNM-81017 and is approved through December 31, 1989. In the event your carrier is required to deviate from the approved route, please call the contact listed on page 2 of this letter as soon as possible providing the details.

Arrangements with law enforcement agencies along the route as required by 10 CFR 73.37 (a)(2) have been completed by the NRC staff. Data relating to these arrange-ments and a copy of the approved route are enclosed. This information is to be incorporated into your shipment plans and provided to your carrier along with instructions regarding its use.

Please note that assuring highway safety is the responsibility of the licensee and carrier and our approval is not intended to provide relief in this regard.

During seasonal periods when inclement weather with accompanying hazardous road conditions can occur with short notice, the appropriate state police should be contacted with regard to road conditions before a shipment commences.

Please note that the notification requirements of 10 CFR 73.37(f) and 73.72 for each shipment still apply. When making a notification to a governor or his )

designee, it should be ury specific especially with regard to notification of l series shipments. Written notification must designate exactly how many individ-ual shipments are to be included. In the event the number of shipments in a series is revised, the change notification should indicate that it is a revision of the 8903030041 390224 PDR XPORT ISNM-81017 PDC

Safe havens are indicated on the route strip charts by a lower case letter of the alphabet in parenthesis. These safe havens were designated by the NRC route analysis teams with the assistance of law enforcement agencies along the route, as available, for tempo-rary refuge until appropriate assistance arrives. The basic criteria used to identify a safe haven were availability of a large parking area, nighttime lighting, where possible, and rapid law enforcement response to the safe haven.

DESCRIPTION OF EVENT on February 7, 1989, with unit 1 in mode 1 (100 percent power 2235 psig, 578 degrees F), and unit 2 in mode 6 (0 percent power, O psig, 80 degrees F),

fire detectors for fire zones 134, 135, 142, 143, 144, and 145 (Auxiliary Building, 714 foot elevation, column line Al-A15 and Q to U) were inoperable. Unit 2 is currently in a refueling outage and is replacing the "B" component cooling water heat exchanger . Replacement of the "B" component cooling water heat exchanger (Workplan No. 12742) involved  ;

intensive welding and grinding. The work area was located in the above I specified fire zones.

To prevent spurious actuation of fire suppression system and spurious fire alarms in the main control room (distraction to the operators) which could occur due to the smoke produced by the welding activity, the fire detectors of the fire zones were removed from service.

Technical specifications (TSs) require that the fire detectors be operable whenever, any equipment that the detector protects is required to be operable. Two other component cooling water heat exchangers are also, located in these fire zones, one of which is required operable for unit 1 operation. Thus, the detectors were required operable. The action statement of Limiting Condition for Operation (LCO) 3.3.3.8 was entered at 2149 EST on January 24, 1989, and a continuous firewatch was established in accordance with the action statement. On February 17, 1989 when welding activities associated with the "B" component cooling water heat exchanger were reduced and the potential for spurious actuation of smoke detectors no longer existed, the smoke detectors were returned to service at 1725 EST.

Thus, the detectors were inoperable for approximately 24 days. The action of LCO 3.3.3.8 requires submittal of a special report if the detectors are inoperable for longet- than 14 days.

CAUSE OF EVENT The replacement of "B" component cooling water heat exchanger was not completed within 14 days due to the complexity of the work involved. To prevent spurious actuation of the fire suppression system and spurious fire alarms in the main control room (distraction to the operators), it was decided to leave the fire detectors inoperable until the smoke produced by the welding activities was reduced.

ANALYSIS OF EVENT This special report is being submitted as required by the action statement (b) of LCO 3.3.3.8.

The fire detectors were inoperable for approximately 24 days. The fire suppression system in the affected areas were operable and would be manually actuated by the operators in case of a fire being detected by the continuous firewatch.

' I Thus, if a fire had indeed initiated in the area protected by these detectors, the mitigating effects of the fire protection system would not have been lessened by the inoperable detectors.

CORRECTIVE ACTION A continuous firewatch was established and was maintained until the smoke l detectors were declared operable at 1725 EST on February 17, 1989. No further action is required l i
