1989, wherein, North American Inspection, Inc. was notified that its Quality Assurance Program Approval for Radioactive Material Package, No. 0514', expires on February 28, 1989.

Jim Nicolosi With Hydro Nuclear Services, Inc.,
was in contact with your(off(ice today and spoke with Mr. Lynn Gordan.
Mr. Nicolosi relayed to Mr. Gordan our delay in submitting for renewal was due to N.A.I.I.'s RSO, Mr. Don Shumway, being away from his office due to a heart attack he suffered in December 1988.
Mr. Nicolosi %s preparing for N.A.I.I.'s NRC license renewal to be submitted this
- week, but this renewal was overlooked.
At the direction f Mr. Gordan, enclosed is our applic'a-tion fee for
$150.00, as required by 10 CFR Pa r t'~ 17 0. 31.
Please accept this let er as our request for renewal..Also,The l
advised that there have not been any substantial changes in N.A.I.I.'s Quality Assurance Program since our 1984 submittal, except for the addition of two (2) possible source suppliers-Source Production & Equipment Co.,
Inc. and RTS Technology, Inc.
Mr.
Nicolosi is in the process of adding these two possible suppliers to N.A.I.I.'s NRC license renewal.
