We have reviewed your new license application dated February 8,1987 requesting a byproduct material license and find that we will need additional information as follows:
1.
Please submit the name of the manufacturer and the model number of both the gadolinium-153 source and the device in which it will be contained.
2.
With regard to source exchange of the gadolinium-153, please address the foliowing:
a.
Please submit the step-by-step procedures you will follow for source exchange.
You may either submit a copy of the manufacturer's procedures or reference them by date.
b.
Please submit the name of the individual (s) that will perform the source exchange procedures, c.
Please indicate that film badges will be worn during source exchange procedu res.
3.
Please submit the name and address of your film badge supplier.
4.
Please provide a sample calculation to demonstrate how leak test results will be converted into microcuries.
5.
Please indicate that you will maintain a copy of the manufacturer's instructions regarding the use of the device on file at your institution.
