3895030820 Attachment to Monthly Operating Report for January 1989 Limerick Generating Station i
Unit 1 January 1 through January 31, 1989 I.
Narrative Summary of Operating Experiences Limerick Unit 1 began the month in Operational Condition 1 at 41% of rated thermal power.
On 1/11/89, reactor power was reduced to a nominal 15% rated thermal power in preparation for Unit l's second refueling outage.
From this power, the main turbine was tripped and the reactor was scrammed.
Cold shutdown was achieved on January 12.
This period ended in operational condition 5 with a reactor coolant temperature at 0
92 F and a reactor vessel level of 480 inches.
Operational evente that occurred during the month of January included:
On January 4, during an engineering review, control and l
power cables associated with RCIC Steam Supply Line Inboard Isolation Valve and also the HPCI system were i
determined not to be in compliance with the Fire Protection Evaluation Report (FPER).
A fire could cause loss of steam supply to RCIC or spurious starts and the inability to shutdown HPCI.
Roving fire patrols were established in the affected areas.
On January 13, RHR shutdown cooling isolated when a fuse blew while a technician was obtaining voltage readings i
from a trip unit.
When the fuse blew, trip units in the associated card file de-energized causing the shutdown cooling isolation.
After replacing the fuse, the isolation was reset and shutdown cooling was restored to service.
On January 17, a discrepancy between normal level indication and indication provided by a temporary level instrument was observed.
Initial evaluation revealed that all normal level instruments were indicating 19" above actual level.
As a result, all Unit 1 diesel generators and ECCS were declared inoperable.
Following satisfactory completion of the backfill procedure, all instrumentation and associated ECCS for Divisions I thru IV were declared operable.
A rigorous investigation was initiated and it was concluded the temporary indication (pressure transmitters) was not accurate until the reactor vessel disassembly has progressed to the point where the vessel pressure is assured to be atmospheric.
II. Challenges to Main Steam Safety Relief Valves There were no challenges to the Main Steam Relief Valves during the month of January.
