Gentlemen

l This is in reference to your request dated February 3,1987 for a byproduct material license. In order to complete our review, we need the following additional information: l l

1. With regard to personnel monitoring devices, you should specify:

(a) Name of supplier (b) Type of devices used (e.g., film badges, TLD)

(c) Frequenc~y of changing monitoring devices. j l l Note that film badges should be exchanged at least monthly and TLDs l should be exchanged at least quarterly.

2. You should provide information concerning your specific instructions to l

the users informing them that any maintenance on the gauges involving dismantling, removal of source holders, etc., must not be performed by the user and must only be performed by the manufacturer of the device.

If you wish to conduct such activities, you must request this specifically and submit step-by-step procedures. You should have a properly calibrated survey instrument on hand if any special maintenance activities will be performed by your personnel.

3. .Your emergency procedures should state that the NRC will be informed whenever a gauge is lost, stolen, or involved in an incident or accident.
