Dear Sirs:
We wish to amend this license as follows:
1.
Please delete Drs. Wilson Weisel, Raymond R. Watson and Thomas M. O'Connor as authorized users, as they are no longer active staff at this hospital.
2.
Dr. John Just remains as an authorized user.
3.
Please add Richard A. Duchelle, M.D., Cardiologist and Ramon E. Acevedo, M.D., Cardiologist as authorized users.
Current license status as requested by Kevin Null of your agency:

Only one (1) Medtronic pulse generator Model 9000, Serial Number 3R00055 which was implanted in 1976, is still under our control.

This patient has been followed for some time by Cardio Care, New York l
which is a division of Medtronics. They have forwarded copies of bi-monthly pacemaker checks to Dr. Acevedo and Dr. Duchelle who also forwarded copies to the patient's primary care physician.
C.
It is intended that beginning October,1987, this patient will be followed
