FERMI 2 1987 Operating Events Review The staff has performed an evaluation of 1987 reported events at Fenni 2 comparing event causes with the objectives of Detroit Edison's Nuclear Operations Improvement Plan (NOIP) and Reactor Operations Improvement Plan (ROIP). A 4/22/87 Detroit Edison memorandum to the staff concerned the implementation status of the NOIP and ROIP, which stated that the ROIP was 90%

complete and that 30 of 47 NOIP objectives had been met.

The staff reviewed Femi 2 LERS received for 1987 events and any additional event reports (50.72, PN) received since 7/20/87. Of the 39 LERs received, 13 were caused by equipment / design factors and 26 were procedure / personnel related.

All 26 procedure / personnel event causes related to ROIP and NOIP objectives, areas which had been previously (1985,1986) identified for increased attention and improvement by Detroit Edison staff. Three significant trends were noted in the staff review, concerns not adequately resolved by the improvement plans:

(1) 18 reported events concerned repeated incidents of mistakes, sloppiness, and cognizant errors made by Operations or I&C personnel at Femi. ROIP objectives (A) and (E), specifically, " increased management support and operator staff effectiveness in the Control Room" and " increased awareness of consequences of errors and strengthened accountability", were designed to reduce these errors.

(2) 9 reported events concerned inadequate procedures, which NOIP objective (C), upgrade existing procedures, was to resolve.

(3) The repeatability of events and event causes evident a lack of adequate effort by the utility to identify and incorporate lessons learned in platet operations, areas to have been resolved by objectives NOIP (8) and ROIP (D).

In conclusion, the staff finds that Detroit Edison has not been effectively implementing the objectives of the NOIP and ROIP.
