PROVISIONAL OPERATING LICENSE NO. DPR-45 SPECIAL REPORT 1B EMERGENCY DIESEL GENERATOR

Dear Mr. Davis:
In accordance with Reference 1, this is to inform you of a diesel generator test failure. Reference 2 requires that a diesel generator must start from ambient conditions and be up to rated bus voltage in less than or equal to 10 seconds.
At 0555 on June 15, 1987, the IB Emergency Diesel Generator was started for a monthly surveillance test.
At 0556, the voltage was observed to be low.
The voltage readings on the three phases were 434 V, 432 V, and 432 V vs. the required voltage of 450 1 5 V.
The voltage regulator rheostat anchor nut was found to be loose. With the nut loose, the rheostat rotated and the voltage adjustment shifted.
The nut was tightened and the voltage adjusted to 450 volts.
Loctite threadlock was used on the anchor nut threads to prevent reoccurrence.
A satisfactory surveillance test was completed at 1120.

The lA Emergency Diesel Generator (EDG) voltage regulator was checked

and found to be mounted securely.
The two diesel generators are of different 1
designs and the voltage regulator installations differ.
The IB EDG is a 400 KW Caterpillar Model D348 engine.
Its last failure I
occurred November 15, 1983 and was related to the starting battery.
The June j
15, 1987 failure occurred during the 111th valid test since the previous
{
failure, on a per nuclear unit basis.
The test interval will, therefore, remain monthly.
The reactor is permanently shut down and defueled. Only one emergency diesel generator is required to be operable during Operating condition 4, Cold Shutdown.
The 1A Emergency Diesel Generator was operable at the time of the IB EDG test failure.
