Please review the' enclosed document carefully and be sure that you understand
all conditions.
If there are any errors or questions, please notify us so -
that'we can provide appropriate corrections and answers.
It should b'e noted that your 1986 Radiation' Safety manual was never submitted
to us in total. Therefore, it is our understanding that the manual is' as -
submitted with your application dated November 11, 1984 and amended in subsequent
letters.
In particular, the section titled " Duties of the Radiation Safety

Committee" and the section titled " Radiation. Safety 0fficer" should. read as

indicated on the enclosed pages.

We also understand that your Radiation Safety Committee now consists.of: Dr.
Edward J. Brown, Dr. Dan Holleman, Dr. George Kipphut, Ms. Dorothy'C. Yates,
