J. M. Farley Nuclear Plant NRC Inspection of April 18 - May 18, 1987 l

Dear Sir:

This letter refers to the violation cited in the subject inspection reports which state:

"During the Nuclear Regulatory Commission (NRC) inspection conducted on April 11 - May 18, 1987, violations of NRC requirements were identified.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the violation is listed below:

TS 6.8.1 requires that applicable written procedures recommended in Appendix A of Regulatory Guide (RG) 1.33, Revision 2, 1978 shall be established, implemented, and maintained.

Administrative Procedure (AP) 16 " Conduct of Operations -

Operations Group" section 3.1.13.5 requires that the designated licensed operator at-the-controls (OATC) shall not under any circumstances leave the at-the-controls area for any reason without obtaining a qualified relief operator.

a.m., the OATC reactor operator position on Unit 1 control board l

was left unattended for approximately 20 seconds. The licensed

Shift Supervisor assumed this position without proper notification j

until the OATC operator returned to the position.

This is a Severity Level IV Violation (Supplement 1)."

The above violation occurred as described in the subject reports.

Reason for Violation The violation was caused by personnel error in that the OATC failed to follow procedure and verify that the at-the-controlf area was properly manned prior to his leaving the at-the-controls area.

Corrective Action Taken and Results Achieved The Shift Supervisor assumed the OATC position. The Unit 1 operators were called and they immediately returned to the at-the-controls area.

Further, a Unit 2 operator (not required to be in Unit 2 at-the-controls area) noticed that the Unit 1 at-the-controls area was unattended by an' operator and he entered the Unit 1 at-the-controls area.

Corrective Steps Taken to Avoid Further Violations A Night Orders entry concerning this event was made and plant operators were required to read applicable sections of AP-16 prior to assuming their next shifts. The Unit 1 OATC was disciplined for his failure to follow procedures and for allowing the Unit 1 at-the-controls area to be left unmanned by a plant operator.
