This error occurs on the original License dated January 12, 1976, on Amendment No. 01 dated February 17, 1981, and on Amendment No. 02 dated September 2, 1983.
Please note that Dr. Bowman's name appears correctly on our original application dated December 17, 1975 and in our letter dated January 8, 1976.
