COMANCHE PEAK STEAM ELECTRIC STATION (CPSES)

DOCKET N05. 50-445 AND 50-446 MISSING WELOS AND UNDERSIZED MEMBERS SDAR: CP-87-71 (INTERIM REFORT)

Gentlemen:

On August 24, 1987, we verbally notified your Mr. H. S. Phillips of a deficiency involving structural steel assemblies which contained undersized welds, missing weids and undersized structural steel members.

These conditions were identified during third party reinspection and are addressed in Corrective Action Request (CAR)87-012.

Our last interim report, logged TXX-6738, was submitted on September 23, 1987. We have concluded this issue is reportable under the provisions of 10CFR50.55(e) and the required information follows.

Description Third party reinspection identified deficiencies involving undersized welds, missing welds and undersized structural steel members in completed structural steel assemblies.

For the undersized members and missing welds, our evaluation has determined that these deficiencies are confined to Category I and 11 structural steel assemblies for which inspection records are not available.

The issue of undersized welds is confined to the AWS welding in the structural steel assemblies.

The cause of this deficiency has been determined to be inadequate engineering specifications when implementing changes to the design requirements in order to comply with Regulatory Guide 1.29.

Safety Implications Undersized welds, undersized members and missing welds could result in failure of the affected components to function as designed during a seismic event.

This issue represents a significant construction deficiency requiring extensive evaluation to establish the adequacy of the affected structures, systems and components to perform their required safety function.

In the event the deficiency had remained uncorrected, the condition could have adversely affected the safety of plant operations.

Corrective Action l

l Seismic Category I and 11 structural steel assemblies for which inspection records are not available will be reworked in accordance with the applicable design drawings.

The reworked assemblies will be inspected in accordance with QI-QP-11.1 4-1, " Inspection of Site Fabrication and Installation of Structural and Miscellaneous. Steel." Any discrepancies will be documented and dispositioned using Nonconformance Reports (NCRs).

Field Verification Method (FVM) CPE-SWEC-FVM-EE-ME-IC-CS-90 has been issued to control the verification of undersized welds.

All undersized welds identified by the FVM will be documented on NCRs, evaluated by engineering and reworked if required to assure structural adequacy. All applicable design drawings, specifications, and the aforementioned procedures have been reviewed and where necessary revised to assure incorporation of engineering requirements.

Corrective actions for Units 1 and 2 are currently being scheduled. Our next report providing the schedule will be submitted no later than March 2, 1988.
