Dear Sir:

This letter refers to the violation cited in the subject inspection reports which state:

"During the Nuclear Regulatory Commission (NRC) inspection conducted on November 2-6, 1987, a violation of NRC requirements was identified.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violation is listed below:

Technical Specification (TS) 6.8.1.c requires that written procedures be established, implemented, and maintained covering surveillance and test activities of safety-related equipment.

Farley Nuclear Plant (FNP) Administrative Procedure, FNP-0-AP-16,

" Conduct of Operation" allows operations memorandum be used as a vehicle for disseminating information or instructions relative to I

the conduct of operations. FNP Intra Department Memorandum dated September 9, 1985, provides instructions for filling out Emergency Diesel Generator (EDG) Test Data Logs in order to comply with NRC Regulatory Guide 1.108 requirements.

Contrary to the above, the EDG Test Data Logs from October 1985 to present were not completed as specified fn the September 9, 1985, Operations Intra Department Memorandum. Examples of omissions include not logging reasons for invalid EDG tests and failures, not logging EDG start times, and not properly logging reasons for EDG starts.

In addition, the Intra Department Memorandum is incorrect in that Criteria B(b) of the memorandum does not comply with NRC Regulatory Guide 1.108 for evaluation of valid and invalid EDG tests and failures. Adherence to this memorandum could result in p(

an impropet evaluation of a EDG test and failure.

This is a Severity Level IV violation.

Admission or Denial The above violation occurred as described in the subject reports.

Reason for Violation The above violation was caused by personnel error. However, it should be i

noted that adequate information was recorded so that any information lacking could be easily retrieved and reconstructed.

Corrective Action Taken and Results Achieved Emergency Diesel Test Data log omissions and errors are being corrected.

Instructions for determining valid and nonvalid failures of diesel generator starts are being revised to alleviate the concerns with auto start failure classifications.

Cctrective Steps Taken to Avoid Further Violations The Emergency Diesel Test Data logs and the associated instructions for filling out data will be revised to improve assessment capabilities for diesel generator valid and nonvalid tests and successful tests and failures.
