[0TICE OF VIOLATION Louisiana Power & Light Company Docket:

50-382 Waterford Steam Electric Statio1, Unit 3 License:

NPF-38 During an NRC inspection conducted on April 28 through May 1, 1987, a violation of NRC requirements was identified.

The violation involved failure to maintain proper personnel exposure records.

In accordance with the " General Statement of Policy and Procedure for NRC fnforcement Actions," 10 CFR Part 2, Appendix C (1987), the violation is listed r.elow:

Failure to Maintain Exposure Records 10 CFR Part 20.401(a), " Records of Surveys, Radiation Monitoring, and Disposal," requires that licensees shall maintain records showing the radiation exposures of all individuals for whom personnel monitoring is required under Part 20.202 of the regulations.

Such records.shall be kept on Form NRC-5, in accordance with the instructions contained in the form.

The doses entered on the forns (or equivalent records) shall be for periods of time not exceeding one calendar quarter.

Contrary to the above, the NRC inspector determined on April 29, 1987, that an individual had received a dose of about 170 millirems during the period July 1 through Septembir 30, 1986, but this dose had not been entered on the individual's exposure record (Form NRC-5 equivalent).

This is considered a Severity Level IV violation.

Pursuant to the provisions of 10 CF? 2.201, Louisiana Power & Light Company is hereby required to submit to this office, within 30 days of the date of letter transmitting this Notice, a written statement or explanation in reply, including for each violation:

(1) the reason for the violations if admitted, (2) corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending the response time.
