Dear Sir:

This report is being furnished in accordance with the conditions stated in NRC License No.

SNM-362.

A leaktest performed on August 22,1997, on a Sr-90 scaled source indicated the presence of greater than 0.005 uCi of radioactive material. The sample contained approximately 0.008 uCi of Sr-90. The source tested was manufactured by Bebig GmbH, serial number 0040/97. The

. source consists of a set of 12 brachytherapy-type seeds with a total Sr-90 activity of 30 mci.

Each seed is cylindrically shaped with dirnensions of 0.6 mm by 2.5 mm.

The leaktest procedure consisted of flushing water through the source storage tube and collecting the water for liquid scintillation analysis. All contamination was limited to the interior of the source storage containers and the leaktest samples.

As corrective action, the source has been withdrawn from service and it is planned to return the source to the manufacturer in the near future. The return shipment will comply with all applicable DOT regulations. The specific cause of the source contamination is unknown at this time.

Previous reports dated August 12,1997 and August 20,-1997, notified you office of leaktest results greater than 0.005 uCi for source numbers 0047/97,0071/97, and 0050/97. Because of this pattern of contamination, increased leaktest surveillance of this model source has been implemented.
