Licensee," (3/98) Enclosure 1, which has been revised to reflect current operator licensing policy. The changes to the form are described in Enclosure 2.
By copy of this letter, your facility training manager is being provided with a small supply of the form and is asked to ensure that medicatinformation is submitted on the revised forms as soon as possible. Copies of the form or reasonable equivalent will be accepted.
