REVISED OPERATOR LICENSING FORM
Dear Mr. Gambhir:
Enclosed is a copy of revised NRC Form 396," Certification of Medical Examination by Facility Licensee," (3/98) Enclosure 1, which has been revised to reflect current operator licensing policy. The changes to the form are described in Enclosure 2.
By copy of this letter, your facility training manager is being provided with a small supply of the form and is asked to ensare that medical information is submitted on the revised form as soon as possible. Copies of ~the form or reasonable equivalent will be accepted.
