For the two subject inspection reports, the NRC indicated that the responses to the

violations shall be submitted under oath or affirmation. Due to administrative error, this
statement was included with the violations, but should not have been. You are required to
respond to the violations; however, this response need not be submitted under oath or
affirmation. We are sorry for any inconvenience this may have caused you.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter
and its enclosures will be placed in the NRC Public Document Room (PDR).
