Dear Sir:
This report is being furnished in accordance with the conditions stated in NRC license No. SNM 362.
A leaktest performed on 8/12/97, on a Sr-90 sealed source, indicated the presence of greater than 0.005 uCi of radioactive material. The sample contained approximately 0.016 uCi of Sr-90. The source tested was manufactured by Be'
'mbH, serial number 0047/97. It consists of a set of 12 brachytherapy type seeds with a total Sr-90 ar
.y of 30 mCl Each seed b cylindrically shaped with dimensions of 0.6mm by 2.5mm.
The leaktest procedure consisted of flushing water through the scurce storage tube and collecting the water for liquid scintillation analysis. Contamination external to the source was limited to the interior of the source storage container and the leaktest sample.
As corrective action, the source has been withdrawn from service and it is planned to return the source to the manufacturer in the near future. The return shipment will comply with all applicable DOT i
regulations. The cause of the source failure is unknown at this time, if you have any questions call Doug Eagleson,301-975-5800, of the NIST Health Physics Group.
