$h Royggh3,l9gg o70 o/3 9V-U.S. Nuclear Regulatory Commission Region III 801 Warrenville Road Lisle, Illinois 60532-4351 Attention: Thomas Young SIIBJECT: Follow-up Report on Explanted Nuclear Pacemaker On Thursday, Febmary 26,1998, a nuclear powered pacemaker, Model 9000, serial number 5R00003, which we implanted on November 25,1975, was explanted at William Beaumont Hospital, Royal Oak, Michigan. The patient had failed to keep the required follow up visits withe his cardiologist afler moving to Traverse City, Michigan in 1995. The Cardiology Section of William Beaumont Ilospital explanted the nuclear powered generator and replaced it with a battery operated generator. The patient is doing well.

The Cardiology staffgave the nuclear powered pacemaker to the Radiation Safety Officer at the completion of the procedure. The nuclear powered pacemaker was packaged and returned to Medtronic Inc. on February 26,1998 via Federal Express Airbill. There has been telephone confirmation that the pacemaker has been received by Medtronic.
