Notice is hereby provided that your Quality Assurance (QA) Program Approval for Radioactive l
Material Packages, Approval No. 0876, expired on November 30,1997.
If you are an NRC licensee and conduct activities undar the General Licenses of Subpart C of 10 CFR Part 71, or an Agreement State Licensee subject to the requirements of 10 CFR Part 71, as required by 10 CFR 9150.20, a OA Program approved by the Commission as satisfying the provisions of Subpart H of 10 CFR _Part 71 is required. Since you no longer have a valid QA Program Approval, the General Licenses of Subpart C of 10 CFR Part 71 and registration ;
under the provisions of 10 CFR 9 71.12, to transport or deliver licensed material to a carrier for l transport are terminated.
Please note that records required by 10 CFR 99 71.91 and 71.135 are to be retained for a period of three years.
