SUBJECT: MILLSTONE UNIT 3 MOTOR-OPERATED VALVE INSPECTION

REPORT NO. 50-423/98-82 AND NOTICE OF VIOLATION j

Dear Mr. Bowling:

This letter transmits the results of the NRC specialinspection conducted on February 9

through February 27,1998 at Millstone Unit 3. The inspection focused on areas important j

to the public health and safety, and consisted of an independent evaluation of motor- )

operated valve (MOV) program procedures, calculations, and test results associated with

NRC Generic Letter (GL) 89-10, " Safety-Related Motor-Operated Valve Testing and l

Surveillance." The preliminary findings were discussed with your staff on February 27,

1998.

Substantial progress was made in the past six months toward closure of the Unit 3

GL 89-10 program. The use of the Electric Power Research Institute's Performance

Prediction Methodology to establish the design basis thrust requirements for over 100

MOVs and the resulting modifications of these valves represent noteworthy efforts. While

the quality of calculations and the technical information for supporting GL 89-10 program

closure were generally good, errors were found in thrust calculations, as well as in the use

of the SMARTBOOKdatabase, affecting practically all MOVs.

The above findings related to calculation errors were characterized as a violation of 10 CFR

50, Appendix B, Criterion lil, " Design Control." The violation is cited in the enclosed

Notice of Violation (Notice) and the circumstances surrounding it are described in detail in

the subject report. ' You are required to respond to this letter and should follow the ,,

'

instructions specified in the enclosed Notice when preparing your response. In your //

response, you should document the specific actions taken and any additional actions you /

plan to prevent recurrence. Your response may reference or include previous docketed

correspondence,if the correspondence adequately addresses the required response. After '

reviewing your response to this Notice, including your proposed corrective actions and the

results of future inspections, the NRC will determine whether further NRC enforcement /

action is necessary to ensure compliance with NRC regulatory requirements, dj

The NRC was unable to close the MOV issues associated with Significant items List

item 26 and reach closure regarding the review of the GL 89-10 program primarily due to

the corrective actions that will be required to address the thrust calculation errors. In

addition, the enclosed inspection report includes nine Inspector Followup ltems and one

Unresolved item. Along with your response to the enclosed Notice, we request that you

address those 10 additional items. For each of those items, we specifically request that

you discuss those interim actions taken or planned in support of GL 89-10 program closure

along with any long-term actions. Your response will facilitate the NRC's subsequent

review of the GL 89-10 program for closure and restart.

The response directed by this letter and the enclosed Notice are not subject to the

clearance procedures of the Office of Management and Budget as required by the l

Paperwork reduction Act of 1980, Pub. L. No.96-511. We appreciate your cooperation, (

and would be pleased to discuss our findings and conclusions. i
