German Church Road Byron,IL 61010 ' BUBJECT: NRC INSPECTION REPORT 50-454/g7015(DRP); 50 455/g7015(DRP) AND NOTICE OF VIOL.ATION Dear Mr. Graesser. ' On September 8, igg 7, the NRC completed an inspection at your Byron Unit 1 and Unit 2 reactor facitMies. The enclosed report presents the inspection results. Based on the results of this inspection, certain of your activities appeared to be in violation of NRC requirements. The inspectors identified three violations. - We are concemed about two of - the violations because of their recuning nature. The first involved the inadvertent start of the 08 essential service water make-up pump during maintenance due to an inadequate work package. A similar event was documented in a previous inspection report when a Unit 2 pressudzor power operated relief valve inadvertenty lifted on March 5,1997, also due to an inadequate work p::,kage. The second violation involved a breakdown of foreign material exclusion controls near the spent ' fuel pool. We have generally considered your foreign material exclusion practices around the spent fuel pool and reactor cavity good. However, a contributor to the breakdown of controls identified during this period was the flexibility allowed in the corporate procedure for foreign- material exclusion controls, inadequate foreign material exclusion practices have also been discussed in previous inspection reports. The third violation concems a failure to implement timey oonective action for a condition adverse [ lo quality, specifically, technical specifications were not amended since December igg 4, to / reflect appropriate setpoints for the auxiliary feedwater system. Although the set controlled administratively, accurate and appropriate technical specifications are vital to the safe operation of the station. The violations discussed above are cited in the enclosed Notice of Violation. -The circumstances surrounding the violations are described in detailin the enclosed report. Please note that you are required to respond to these violations and should fo; low the instructions specified in the enclosed Notice when preparing your response. The NRC will use your response, in port, to determine whether further enforcement action is necessary to ensure compliance with regulatory c requirements, D J q . . . .
