SUBJECT: NRC INSPECTION REPORT 50-416/97 16
Dear Mr. Hagan:
An NRC inspection was conducted September 29 through October 3,1997, at your Grand
Gulf Nuclear Station reactor f acility. The inspectic-) reviewed your licensed operator
requalitication program. The enclosed report presents the scope and results of that
inspection.
During this inspection, we observed teamwork and communication breakdowns on the
operating crew evaluated. We also noted individual problems of a comparable nature had
been observed by your staff during annual examinations the week before this inspection.
As a result, we requested the remediation plan for the individuals anri crew observed during
the inspection and a summary of the cycle overall results after you have completed the
annual examination cycle, at the end of the month. We understand you plan to provide this
ininrmation fo our review, if this does not reflect your position, please let us know,
in accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter
and its enclosure will be placed in the NRC Public Document Room (PDR).
Should you have any questions concerning this inspection, we will be pleased to discuss
them with you.
