Dear Mr. Pluister:
This acknowledges receipt of NRC Form 241 dated January 19,1998, and the $1,100.00 application fee, which you submitted to report proposed activities in NRC jurisdiction under the authority of the generallicense pursuant to 10 CFR 150.20. Your Agreement State
' License No. IL-01354-22 is recognized as valid for the proposed use of licensed material as described in this initial application not to exceed 180 days in calendar year 1998.
Deletion of specific work locations or work sites, work site contacts, addition and/or deletion of dates of work, or changes in the equipment to be used when there is no change in the activity to be conducted or the radioactive material to be used are considered clarifications and as such, do not require a fee.
Information submitted on the Form 241 must be specific regarding the location and dates of work. When submitting clarifications of proposed dates and locations of work, include the Location Reference Number assigned to a particular work location. This information can be found on the approved NRC Form 241, attached. Transmission of clarifications by

facsimile is considered acceptable. To ensure appropriate accountability of your work activities, you should confirm via telecon that NRC has received all facsimiled information.
On March 21,1997, Massachusetts became an Agreement State. Therefore, work at Epic Enterprises, Ayer, Massachusetts, does not require reporting to the NRC and has been deleted from your NRC Form 241.
A $200.00 fee is required whenever you revise the initial 241 form to include additional locations of work; when you propose to use different radioactive materials; or you plan to perform additional work activities not previously approved by NRC.
