TASK ORDER NO. 123, ENTITLED, "0VERSIGHT Of THE HILLSTONE EMPLOYEE SAFETY CONCERNS PROGRAM" UNDER CONTRACT NO. NRC-03-93-026

Dear Mr. Lofy:

In accordance witn Section G.4, Task Order Procedures, of the subject contract, this letter confirms the verbal authorizations provided on July 10, 1997, and July 15,1997, and definitizes the subject task order.

The effort shall be performed in accordance with the enclosed Statement of Work and the Contractor's technical proposal dated July 1, 1997.

Task Order No.123 shall be in effect from July 10, 1997 through June 30, 1998 with a cost ceiling of $151,421.39.

The amount of $147,306.00 represents the estimated reimbursable costs, the amount of $138.13 represents the facilities capital cost of money, and the amount of $3,977.26 represents the fixed fee.
