SYSTEMATIC ASSESSMENT OF LICENSEE PERFORMANCE (SALP)

REPORT 50-416/97-99 Dear Mr. Hagan.

Enclosed for your review is the SALP report for the Grand Gulf Nuclear Station, for the period February 25,1996, through September 6,1997. A public meeting to discuss this l

report with you and your staff has been scheduled at the Grand Gulf Energy Center Auditorium on October 29,1997, at 9 a.m. (CST). During this meeting you are encouraged and oxpected to candidly comment on this report. Although this meeting is a forum between Entergy Operations, Inc., and the NRC, it will be open to observation by members of the public and other interested parties.

l Your performance during this rating period continued to demonstrate Er.tergy's commitment to the safe and conservative operation of the Grand Gulf facility. The Operations and Engineering areas were again determined to be performing at a superior (Category 1) level, in the Maintenance area, problems identified in your previous SALP were effectively addressed, and performance was judged to have improved to a suprior (Category 1) level. Overall performance in the Plant Support area was rated as good (Category 2). This reprosents a decline from the superior rating assigned in the previous SALP.

The superior rating in the Operations area was highlighted by the excellent performance of the operators during abnormal and rionroutine events, and by management's safety-conscious decisionmaking. Maintenance performance was characterized by the improved material condition of the f acility and the significant reduction of maintenance related transients. The superior rating in the Engineering area was based, in large measure, on the high quality of the systems engineering func,tiran, the continued emphasis on training within the entire engineerin0 organization, and the strength of engineering self assessments.

The decline in the Plant Sul: port area was primarily based on continuing performance weaknesses in emergency preparedness; however, a decline was also noticed in the implementation of radiological controls practices by both radiological control technicians and radiation workers, In the emergency response areas, the corrective actions which you initiated in response to your previous SALP report were effective in addressing those specific i: sues; however, additional areas of weakness were identified during the

programmatic inspection conducted during this rating period. Problems noted during your September 1997 biennial exercise further confirmed the implementation difficulties you are experiencing in this area.

The effective conduct of self-assessments was evident in several areas during this rating period. However, the Board agreed with your assessment that Grand Gull needed to initiate more broadly focused self assessmeats; also, the peer visits which you conducted at other utilities were recognized as providing valuable insights for your overall perfntmance improvement iaitiatives.

The SALP Board identified a consistent performance theme during its deliberations.

Inattention to detail (1) by individual wnrkers in the conduct of their duties, (2)in the implementation of programs and procedures, and (3)in interorganizational communications was seen across the functional areas. While the individual examples were not safety significant, they did indicate the need for continued managemont involvement.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Title 10 Code of Federal Regulations. a copy of this letter and the SALP report will be placed in the NRC's Public Document Room.
