NRC INTEGRATED INSPECTION REPORT 50-309/97-09 AND NOTICE OF

VIOLATION

Dear Mr. Meisner:

On March 7,1998, the NRC completed an inspection at your Maine Yankee reactor facility. Tt',e

results of that inspection were discussed with you and others of your staff at an exit meeting

held on March 17,1998. The enclosed report presents the results of that inspection.

During the three-month period covered by this inspection period, your c,onduct of activities at the

Maine Yankee facilities was safety focused. Operations associated with the primary system

decontamination were well controlled and this radiologically significant evolution was performed

in an excellent manner. Planning and implementation of radiological controls were very good as

evidenced by thorough and detailed as-low-as-is-reasonably achievable (ALARA) reviews, close

health physics oversight, and substantive and sound radiological controls.

Several instances of poor worker performance were noted during this inspection period.

Examples include poor performance and oversight during the reactor cavity seal test and

several examples of poor performance during radiological work. Several of the examples

involved contract personnel. Adequate supervision of contract personnel continues to be an

ongoing issue at Maine Yankee.

These issues are cited as violations in the enclosed Notice of Violation and the circumstances -

surrounding the violations are described in detail in the enclosed report. Please note that you

are required to respond to this letter and should follow the instructions specified in the enclosed

Notice when preparing your response. The NRC will use your respr ase, in part, to determine

whether further enforcement action is necessary to ensure compliance with regulatory

requirements.
