NITACIIMENT A, " Response to Notice of Violation" ESK-97-147 Page1ofI STATEMENT OF VIOLATION (NRC IR 50-254/97006-02): NOTICE OF VIOLATION Technical Specification (TS) 6.8.A required applicable procedures recommended in Appendix A of Regulatory Guide 1.33, revision 2, February 1978 be implemented. Appendix A, paragraph 3.W references procedures for surveillance tests of the emergency power system. Quad Cities operating surveillance (QOS) procedure 6500-3, step C.I.t required the operator to verify, at the 901-8 panel, that busses 14-1 and 24-1 tie breaker was closed. Coricary to the above, on April 15,1997, Operators did not properly implement procedure QOS l 650m3,4 KV Bus 14-1 Unden'oltage Functional Test. The operator initialed step C.I.t as l completed but failed to verify that the breaker was closed. The breaker was in the open position. This is a Severity Level IV violation (Supplement IV). REASON FOR VIOLATION: Comed acknowledges this violation. The reason for the event was poor attention to detail by the operator who performed the prerequisite step C.I.t. The operator did not fully read the step and mentally completed the sentence to read "open" instead of" closed" as stated in the similar prerequisite step C.l.s of the procedure. The operator then proceeded to the 901-8 panel and verified that busses 14-1 and 24-1 tie breaker was open. ACTIONS TAKEN: The Operator who made the error was counseled on the event. ACTIONS TO PREVENT FURTMER OCCURRENCE: There are no further actions required for this event. DATE WIIEN FULL COMPLIANCE WILL HE MET: Full compliance was met on April 16,1997 when the affected breaker was properly positioned and the suneillance was successfully completed. a}}
