SUBJECT: NRC INSPECTION REPORT 50-346/97009(DRP); NOTICE OF VIOLATION
Dear Mr. Wood:
On August 18,1997, the NRC completed an inspection at yout. Davis Besse site. The
enclosed report presents the results of that inspection.
Two violations of NRC requirements were identified during this inspection. The fir'.t violation
involved a " Conduct of Operations" procedure that granted the shift supervisor unilateral
authority to allow procedural steps to be performed out-of order under certain conditions.
This authority is not consistent with the requirements of the Davis Besse Technical
Specifications that require, for a procedure change, a second review and approval by
another member of your management staff. This violation is of particular concern because
the temporary procedure change process was circumvented by an approved procedure,
indicating a lack of staff understanding and appreciation for Technical Specification
requirements.
The second violation involved a failure to .mplement adequate interim corrective actions
associated with a probabilistic safety assessment report for a Circulating Water system
linebreak. The probabilistic safety assessment report, issued May 1996, identified that
operations personnel would have insufficient time to respond to a worst case Circulating
Water linebreak before the resultant flooding caused a complete loss of feedwater, including
auxiliary feedwater. While Davis Besse's staff took substantial corrective action to reduce
the probability of the event occurring, little was done in the way of immediate actions to
minimize the consequences of the accident.
These violations are cited in the enclosed Notice of Violation, and the circumstances
surrounding the violations are described in detailin the enclosed report Please note that

you are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your resporse. The NRC will use your responso, in part, to
determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements.
