Request for Approval of a Conditional License for a Licensed Operator
Dear Mr. Beach:
By letter dated May 16,1997 (Serial Nu'nber 1-1127), Toledo Edison notified the Nuclear Regulatory Commission (NRC) that licensed operator, Mr. Maf.in J, Migot, Docket Number 55-31272, License Number OP-30630-1, was on medical leave with a potentially disqualifying medical condition and that Toledo Edison would notify the NRC upon a change in status.
Mr. Migot's medical condition was di.; cussed with Ms. Mary Ann Dies, NRC Region 111 Operating Licensing Assistant, on June 19,1997. As discussed in the June 19 phone call, Toledo Edison requests the NRC's approval of a cor.ditional license for Mr. Migot in accordance with 10 CFR 55.25 and 10 CFR 55.33. A revised Medical Certification Form (NRC Form 396) and additionel information regarding Mr. Migot's medical condition are being submitted. Details

conceming the medical condition are considered confidential and exempt from public disclosure in accordance with 10 CFR 2.790 (a)(6).

duties with a "no solo" (i.e., another individual must be present when the individual is

performing licensed duties) stipulation administratively in place while the NRC was evaluating.

. Mr. Migot returned to work July 7,1997. An examination performed by Toledo Edison's physician consultant and an evaluation by his supervisor verifying his ability to perform licensed duties were completed. Mr. Migot meets the "Cenditions oflicensees" set forth in 10 CFR 9700070319 970731 PDR ADOCK 05000346.
