NRC INSPECTION REPORT NO. 50-254/98004(DRP); 50-265/98004(DRP) AND
Dear Mr. Kingsley:

This refers to the inspection conducted February 11 through March 31,1998, at the Quad Cities
Nuclear Power Station. The purpose of the inspection was to determine whether activities
authorized by the license were conducted safely and in accordance with NRC requirements. The
NRC conducted routine resident inspections in this period. Based on the inspectors' findings,
overall performance at the Quad Cities Station was safe. At the conclusion of the inspection, the
findings were discussed with those members of your staff identified in the enclosed report.
Based on these and other findings of this inspection, the NRC has determined that five violations
of NRC requirements occurred. The violations are cited in the enclosed Notice of Violation
(Notice) and the circumstances surrounding them are described in detail in the subject inspection
repod.
The first two violations involve problems with maintaining control of system configuration. One
was caused when an operator failed to ensure a station blackout diesel output breaker was
charged when it was retumed to service. This error was not identified by operators on
subsequent rounds. The next violation involved failure of maintenance personnel to effectively
control contractor personnel which led to a contractor operating out-of-service tagged equipment.
The last three violations involve failure to accomplish Technical Specifications-required
surveillances. These are of concem because although you identified these problems, previous
corrective actions for similar violations have not been fully effective. One violation involved an

improper setpoint used to ensure standby liquid control temperatures prevent boron precipitation.
The next violation involved a breakdown in control of administrative processes used to implement

surveillance testing. (This violation was of concem because it resulted in all source range
nuclear instruments for both units being inoperable in the shutdown mode when they were
required.) The last violation involved a failure to test and inspect piping suppod snubbers on
time which ultimately rendered plant safety equipment inoperable.
You are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC wiu use your response, in part, to
determine whether further enforcement action is necessary to ensure compliance with regulatory
requirements.

In your response you should address actions taken to ensure the rigor of configuration control
processes including out-of service tagouts. In addition, you should address why previous
corrective actions for surveillance violations were ineffective, and what new methods will be in
place to ensure that corrective actions taken are working effectively.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, its
enclosures, and your response will be placed in the NRC Public Document Room.
