SUBJECT: NOTICE OF VIOLATION AND NRC INSPECTION REPORT

NO. 50-254/98005(DRS); 50-265/98005(DRS)

Dear Mr. Kingsley:

This refers to the inspection conducted February 16,1998 through March 26,1998, at the Quad

Cities Nuclear Power Station. The purpose of the inspection was to review design modification

process activities in use at the station. Specifically, the use of emergency and immediate

modifications as well as use of at-risk work activities were reviewed. At the conclusion of the

inspection, the findings were discussed with those members of your staff identified in the

enclosed report.

The inspection concluded that development of emergency and immediate modifications was

allowed by the Quad Cities OA Program, and corporate, and station procedures and was

adequately controlled. With regards to at-risk work packages, satisfactory controls were in

place to ensure that affected components would not be declared operable until completion of all

design documentation. Use of these types of modifications and work packages was not

common and, when used, adequate programmatic controls existed, with one exception noted

below, to ensure minimum prerequisites, limitations, and operational restrictions were included.

Notwithstanding the overall adequacy of the processes and controls, the NRC noted that when

the at-risk work option was introduced in 1996, all applicable design modification procedures

were not revised. Specifically, QCAP 0460-01," Plant Design Changes Process," continued to

require that installers use approved design change packages prior to work package /

'

preparation. Detailed reviews of at-risk work packages revealed an example of a work package

that was prepared contrary to the procedural requirements of QCAP 0460-01 and was

considered a violation of NRC requirements. The violation is cited in the enclosed Notice of l

Violation (Notice) and was of concern because it highlighted an inadequate review of

procedures when implementing an important change to the design modification process.

You are required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response. The NRC will use your response, in part, to

determine whether further enforcement action is necessary to ensure compliance with

regulatory requirements.
