The Confirmatory Order establishing the Independent Corrective Action Verification Prograin for Millstone Units 1, 2, and 3, dated August 14, 1996, and the associated Commission Paper (SECY 97-003) released January 30, 1997, require that the review in: hide a sample of changes made to the facility configuration since issuance of the operuting licens3 through processes other than the design change processes. In order to establish a criteria for selecting these samples, it was mutually agreed that a listing of facility changes in each area of review would first be obtained from NU. In our letter dated July 22,- 1997, we provided sample selection criteria for the first four review areas.

Change lists have been provided to S&L for the following four additional areas:

2. Like-for Like Replacemants.

Commercial Grade Dedications-600 (1986 to 1997)

To establish the ICAVP sample, select:

1. Distribution of dedications from among various types, e.g., mechanical, electrical, IIVAC, computer software.

2. From printout of dedications sorted by year, two dedication packages per year (total 22).

To establish the ICAVP sample, select:

Programmatic Review - Past Change Review Selection Criteria NCRs/EWRs Dispositioned Use-As-Is e Millstone Status:

Approximately 1,480 NCRs throughout the plant systems listed with use-as is disposition (1985 to 1996).

To establish the ICAVP sample, select:

1. NCRs from safety / risk significant systems outside the ICAVP sample systems including r.on-system related NCRs

2. NCRs relating to Section XI requirements (if noted in NCR description).

3. NCRs that would appear to involve an EWR to perform an engineering assessment.

4. Ten NCRs from each five-year interval of plant operation (total 20).

Master setpoint list includes approximately 3,000 setpoints.

To establish the ICAVP sample, select:

1. Setpoints in systems that are safety / risk significant outside the ICAVP sample.

2. Setpoints for equipment typically located in harsh EQ zones.

3. Ten setpoint changes from each five-year interval of plant operatiou (total 20).
