SUBJECT: NRC INSPECTION REPORT 50-445/98-02; 50-446/98-02 AND
Dear Mr. Terry:
From February 15 through March 28,1998, an inspection was conducted at your Comanche
Peak Steam Electric Station, Units 1 and 2 reactor facility. Following the exit meeting, the
results of this inspection were discussed with Mr. Walker of your staff. The enclosed report

presents the scope and results of that inspection.

During the 6-week period covered by this inspection period, activities at the Comanche Peak
facility were generally characterized by safety-conscious operations. However, four violations
were identified during this inspection period. The first violation involved a failure to adequately
control activities in the switchyard. This violation is being cited because your corrective actions
were neither prompt nor effective. The'second violation involved a failure to follow procedures
during new fuel receipt. The third violation involved a failure to identify that operating the Unit 1
safety injection pump motors at an increased frequency may have caused this equipment to
i
exceed the qualification design bases. The fourth violation involved a failure to maintain the
Units 1 and 2 containment doors locked as required by Technical Specifications.

The fourth violation does not reflect concerns with current performance. The NRC has
concluded that information regarding the reason for the violation, the corrective actions taken
t
and planned to correct the violation and prevent recurrence are already adequately addressed in
l
the enclosed report. Therefore, you are not required to respond to this violation unless the
description herein does not accurately reflect your corrective actions or your position. In that
i
case, or if you choose to provide additional information, you should follow the instructions
]
specified in the enclosed Notice.

The violations cited in the enclosed Notice of Violation (Notice) and the circumstances

surrounding the violations are described in detail in the enclosed report. Please note that you

are required to respond to this letter and should follow the instructions specified in the enclosed
Notice when preparing your response. The NRC will use your response, in part, to determine

whether further enforcement action is necessary to ensure compliance with regulatory
requirements.
