NRC INTEGRATED INSPECTION REPORT 50-334/98-01,50-412/98-01

NOTICE OF VIOLATION

Dear Mr. Cross:

The NRC conducted an inspection of the Beaver Valley Power Station (BVPS) facilities from

,

February 8 to March 21,1998. The enclosed report presents the results of that

inspection.

During the 6-week period covered by this inspection report, your conduct of activities at

the Beaver Valley Power Station facilities was generally characterized by safe operation.

The decision to maintain the units shutdown while resolving additional Technical

l

Specification (TS) deficiencies was appropriate. Following the shutdown, your staff

acknowledged the broad scope and significance of the TS surveillance testing problems,

which resulted from poor TS quality, a long-standing non-conservative philosophy .

regarding TS interpretation, and wide spread knowledge deficiencies regarding TSs.

Corrective actions were under development at the close of the inspection period.

Our inspectors observed that your staff had difficulty planning and working maintenance

activities according to schedule and that operational post maintenance testing control and

planning were poor. Combined, these two issues created unnecessary operational burden

i

on the control room staff and increased equipment out of service time. Your ongoing

corrective actions in these areas warrant increased management attention.

You have established generally effec'tive programs for internal and external dosimetry

measurement and record keeping, including respiratory protection. Generally effective

controls over radiological work being performed, especially in the Unit 2 containment

building was also observed.

Based on the results of this inspection, the NRC has determined that two violations of NRC

requirements occurred. These violations are cited in the enclosed Notice of Violation

- (Notice) and the circumstances surrounding them are described in detail in the subject

-inspection report. The violations are of concern because they were identified by the NRC.

In the first instance, you failed to promptly address a known TS deficiency (degraded

voltage setpoints) in a timely manner. And in the second instance, you performed a

- modification on the ground overcurrent trip isolation feature for the Unit 2 emergency

-

diesel generators without fully evaluating all failure modes of the existing configuration.

enclosed Notice when preparing your response. The NRC will use your response, in part,

to determine whether further enforcement action is necessary to ensure compliance with

regulatory requirements.
