The licensee shall assure that the shutter mechanism is locked in the closed position a y during periods when a portion of an individual's body may be subject to the direct a p radiation beam. The licensee shall review and modify as appropriate its " lock-out" g y procedures whenever a new gauge is obtained to incorporate the device manufacturer's y p recommendations. y ' h6. Sealed sources containing licensed material shall not be opened or sources removed from source holders by the licensee. l q y U l7. A. Sealed sources shall be tested for leakage and/or contamination at intervals not to 5 exceed the intervals specified in the certificate of registration issued by the NRC l g l under 10 CFR 32.210 or by an Agreement State. , y B. In the absence of a certificate from a transferor indicating that a leak test N has been made within the intervals specified in the certificate of registration issued by the NRC under 10 CFR 32.210 or by an Agreement State, prior to the l y transfer, a sealed source received from another person shall not be put into use until tested. l I C. Sealed sources need not be tested if they are in storage and are not being used. N 1 However, when they are removed from storage for use or transfer to another person, N I. and have not been tested within the required leak test interval, they shall be N tasted before use or transfer. Possess radioactive material only in the quantity and form indicated in your license. 3. Use radioactive material only for the purpose (s) Indicated in your lleense. 4. Notify NRC in writing of any change in mailing address (no fee required if the location of radioactive material remains the same). 5. Request and obtain written NRC consent before transferring your license or any right thereunder, either voluntarily or involuntarily, directly or indirectly, through transfer of control of your license to any person or entity. Per your telephone question on human occupation near the prospective sourecs. The most occupied location would be the control room. There is 24 hour occupation at the control consoles. These consoles are 65 feet ma ny from the nearest source (120 feet from the further source) and about 45 fret lower (21/2 lhior levels). Also there are at least four permanent walls between them. Officci, occupied during the regular 40 hour work wock, are from $0 feet to 100 feet away hortrontally and 40 feet vertically. Bathroems and a lunch room are the nearest areas subject ruatine human occupation. These are approximately 30 feet away horirontally through a concrete block wall, and 40 feet (two floors) lower, through concretc and steel flooring. 1he areas immediately (l$'3") below the sources are not expected to be frequented. All routinely operated devices are powered and controlled from the control consoles, therefose this area is typlea!!y subject only to penod inspecuens and maintenance (such as the annual feeder gearbox oil change). l Please see attachment. The sources are located in Healy Unit Two approximately
15 feet above the fourth floor, between the elevator and the south office stairwell. Item 2, details on access to gauges: j There are no platforms, ladders, or ready means of access to the new 100 mci source locations. They are paced about 15 feet 3 inches above the feeder deck, and over equipment. Walkways are from approximately 5 feet to 12 feet away horizontally at the
j floor level. The feeder platform will be accessed for brief periods for feeder inspections
by plant operators. During plant overhauls mechanical and electrical crews may work on the feeders but, in our experience no one has worked in a bunker downspout, as in, in the beam path, for any reason, ever The downspouts are designed to last the entire life of the plant therefore access should never be necesmy Even so labels and signs, similar to those in place by the existing sources, "'ill b. placed to caution people not to work in the area without locking the shutters closed on the source housings. Addition of Part B: Part B: Two additional Cesium 137 sources will be used, similarly to the existing gauges, to measure the level of coal in a pipe. There are two 24" diameter coal feed pipes running from two coal storage silos in the Healy Clean Coal Project (HCCP). IICCP is a new coal fired power plant built adjacent and attached to Healy Unit 1, effectively being Healy Unit 2, in a connected structure with several common systems including coal handling, if a low level is detected by the gauges v!brators are activated on the silo to break free bridged or stuck coal, thus restoring coal now automatically. See attached drawing D29404 for the complete arrangement. Amendment to Item 7. Addition of Part B: Part B: Golden Valley Electric Association (GVEA) will use licensed contractors for such parts of the leak testing procedure that require use of licensed personnel, GVEA has established a procedure for swabbing the source housings, for use in conjunction with leak test kit manufacturer's procedures, such that GVEA personnel will perform the swabbing, under direct supervision of the Radiation Protection Officer (Paul Morgan), and the swabs will subsequently be examined appropriately in a properly licensed laboratory. Note that this amendment supersedes the original application in so far as GVEA no longer wishes to be limited to the use of Kay-Ray /Sensall for leak testing. Amendment to item 9.
