Dear Mr. Orser:

This refers to the special NRC team inspection conducted by Messrs. B. Drouin,

M. Dapas, E. Plettner, S. Reynolds and Ms. M. Miller at the Fermi 2 site on

April 29 thrcugh May 10, 1991, and the NRC in-office review of May 23 and 31

and June 3, 1991, concerning the fermi 2 modification implementation process.

This inspection covered modification activities at Fermi 2 authorized by

facility Operating License No. NPF-43. Our findings were discussed with you

and others of your staff at the conclusion of the onsite inspection on

May 10, 1991, and with Mr. R. Newkirk of your staff on June 10, 1991.

The enclosed copy of our inspection report identifies areas examined during

the inspection. Within these areas, the inspection consisted of a selective

examination of procedures and representative records, observations and

interviews with personnel. The primary emphasis of the inspection was to

conduct a performance-based evaluation of modification work activities.

The overall process for the design, implementation, and testing of

modifications was adequate. Management planning, engineering design package

preparation, field implementation, supervisory oversight, engineering support,

and craft knowledge all were good. However, detracting from the overall good

performance were numerous minar documentation errors, work steps occasionally

performed nonsequentially as called for in the work procedures, and several

instances of cortractor personnel failing to follow proper radiological

controls.

During this inspection, certain of your activities appeared to be in violation

of HRC requirements, as specified in the enclosed Notice. A written response

is required.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of

this letter, the enclosures, and your response to this letter will be placed

in the NRC Public Document Room.
