Oil Well Logging Source DESCRIPTION:
Americium oxide powder and beryllium powder are mixed in an inner l
stainless steel capsule sealed by an end plug and a continuous inert gas weld and having a minimum wall thickness of 0.045 inch l
(1.14 mm).
The inner capsule is sealed in an outer stainless steel capsule having a minimum wall thickness of 0.025 inch (0.64 mm) by use of an end plug and a continuous inert gas weld.
The bottom of the source (plug-end) includes a 1/4-20 thread tap l
0.200 inch ( 5. 0 8 mm) deep in the 0.250 inch (6.4 mm) thick plug.
l Overall length of the capsule is 2 inches (50.8 mm) and overall diameter is 0.75 inch (19.05 mm).
LABELING:
The model and serial number of each source is stamped in the bottom of the source.

SAFETY ANALYSIS
SUMMARY
The PL-104 Oil Well Logging source was originally approved for licensing purposes by the Atomic Energy Commission (AEC) on l
December 3, 1971.
This source is no longer manufactured and distributed by Parkwell Laboratories.
The company is out of business.
There is insufficient information on file for this source to have performed a complete safety evaluation.
Therefore, this registration certificate has been transferred to inactive status without further review based on the original review and approval issued by the AEC.

Insocction on January 21. 1993 (Recort No. 030-05868/93001(DRSSH Areas insoected: This was a special closcout inspection which included a review of the licensee's former activities associated with the decontaminatio ^ and remediation of their formerly used office, manufacturing, and laboratory c areas located at 429 North High Street, Croton, Ohio. This inspection was in response to an NRC project,in which an evaluation of approximately 17,000 retired licenses was conducted by an NRC contractor, Oak Ridge National Laboratories (ORNL). On the basis of the information in the retired license files, sites having a potential for significant residual contamination in their former facilities were to be investigated further. Due to the lack of documentation in the General Nuclear, Inc. and Parkwell Laboratories, Inc. retired license file, the NRC contractor concluded that the former facilities f,' used by these companies during the period of 1965-1980 could have significant amounts of residual contamination. Results: The NRC inspector did not identify any radiation levels above natural background within the facilities or surrounding environment. Based %pg/--5 0 / + .I.. .__ 9GO62pe&49 930611 E',i(, c'.'
