Modification No. 5 to Task Order No. 07 Under Contract No. NRC-39-89-133 The purpose of this modifications is to:
(1) increase the level of effort for data entry support, (2) extend the period of performance through June 30, 1992, (3) increase the task order ceiling amount by $73,606.31 from
$120,896.42 to $194,502.73, and (4) increase the task order obligated amount by $30,000.00 from $120,896.19 t, $150,896.19.
Accordingly, the task order is hereby modified to read as follows:
1.
The third paragraph and the first sentence of the fourth paragraph of the definitization letter for this task order are revised to read as follows:
"lask Order No. 07 shall be in effective from February 1, 1989 through June 30, 1992 with a cost ceiling of $194,502.73.
The amount of
$182,188.95 represents the estimated reimbursable costs and the amount of $12,313.78 represents the fixed fee.
The amount presently obligated with respect to this task order is
$150,896.19 of which $141,158.62 represents the total estimated reimbursable costs and the amount of $9,737.57 represents the fixed fee."

As a result of this extension through June 30, 1992, the level of effort for this task order is hereby increased by 3,900 from 6,800 to 10,700 of regular hours.
