MODIFICATION NO. 4 TO TASK ORDER NO. S UNDER CONTRACT NO. NRC-33-93-225 The purpose of this task order modification is to extend the period of performance of the subject task order from September 30, 1997 to December 31, 1997, to allow for continuation of work as set forth in the enclosed Statement of Work, at no additional cost or obligation to the Government beyond the current task order ceiling amount of $284,363.75.
The issuance of this task order modification does not amend any other terms or conditions of the subject contract.
Please indicate your acceptance of this task order modification by having an official authorized to bind your organization execute three copies of this document in the space provided and return two copies to Ms. Brenda J. DuBose, contract Specialist.
You should retain the third copy for your records.

.identifie. the steps involved to add / modify / delete forms images from the servers as well as how to maintain the new 4.1 menu so that it may reflect these changas.
The user of this guide will be the forms administrator. The form administrator, while he/she may not be necessarily technical, must be able to determine frcm this guide the type of technical support staff to use in order Oc affec the desired changes.
This guide also must identify any steps required by the forms administrator with respect to possible changes to users' drives and how these changes are te be handled.
A draft version of the guide shall be submitted to the NRC Task Manager 30 days prior to the final delivery date, o
The Scope and Analysis Document (describing application /a required

The contractor shall implement the recommended automated process as described in the Scopa and-Analysis-document, The contractor shall previde one U.) hard copy and an electronic
- copy, where appropriate, of the systems documentation such as source code, listings, macro language used, user's guides and systems manual as appropriate.
The contracter shall provide up to two (2) demonstrations of 4.1 conversion u llity and installation of the 4.1 menu.
o The contractor shall provide the NRC with three (3) hard copies of all instructions and one (1) copy of the sof tware on disk to enable NRC to implement the conversion utility and 4.1 menu without contractor support.

o The centractor shall automate and implement up to 50 more forms (251-300), as directed by the NRC Task Manager,'using the INFCRMs fesigner tool chosen by the NRC.
These forms will contain ful-1 intelligence (e.g.,
- edits, cross
- edits, required / optional conditional fielde, etc.).
Contractor is tasked to work directiv with Information Records Management Branch (IRMB) to automate these forms.
TRAVEL It is anticipated that local travel from the contractor's Silver spring.
Mnryland office to the NRC Headquarters (Rockville. Maryland) leescion will ba required on a daily basis for performance of work under this task Order.
