SUBJECT: NRC INSPECTION REPORT 50-382/98-06 AND NOTICE OF VIOLATION
Dear Mr. Dugger:
From February 1 through March 21,1998, an inspection was conducted at your Waterford
Steam Electric Station, Unit 3 reactor facility. The enclosed report presents the scope and

results of that inspection.
During this inspection, three violations of NRC requirement were identified. The first violation
l
involved the failure to test a containment isolation valve (CVC-103) after performing
maintenance to ensure that the velve could still perform its intended safety function.
The second violation involved a failure to perform comprehensive corrective actions following
l
overflow of the spent fuel pool. Several other diaphragm valves with deficiencies were identified.
l
and as a result, it was determined that the scope of your staff's corrective actions was narrowly
l
focused.
The third violation involved an engineer performing an operational activity without the direction or
concurrence of control room personnel. This was a repeat of a similar violation that occurred in
early 1996.
These violations are cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding the violations are described in detailin the enclosed report. Please note that you

are required to respond to this letter and should follow the instructions specified in the enclosed

Notice when preparing your response. The NRC will use your response, in part. to determine

whether further enforcement action is necessary to ensure compliance with regulatory
l
requirements.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, its
enclosures, and your response will be placed in the NRC Public Document Room (PDR).
