SEQUOYAH NOCLEAR PLANT REQUALIFICATION EXAMINATION REPORT NO. 50-327/0L-91-01 The NIC administered retake examinations on February 20, 1991, to employees of your company who cuirently hold licenses to operate the Sequoyah Nuclear Pl ant. No requalification program evaluation was conducted.
At the conclusion of the examination, preliminary findings were discussed with those members of your staff identified in the enclosed report.
One finding is of particular concern to us.
In two of three simulator scenarios evaluated, Senior Reactor Operators delayed accident mitigation by setting artificial limits on plant parameters, in one case this lengthened the duration of an off-site release.
Details of these actions are in the Exit Interview paragraph.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter and its enclosures will be placed in the NRC Public Document Room.
