NRC INSPECTION REPORT NO. 50 373/97007; 50 374/97007 AND

- NOTICE OF VIOLATION

Dear Mr. Suba!usky:

This refers to the inspection conducted on May 10 through June 27,1997, at the LaSalle

f acility. The enclosed report presents the results of that inspection.

During this 7-week inspection period, your staff's performance on three occasions

demonstrated a non conservative approach to equipment operability. Specifically,

operations shif t management waited 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> to declare a residual heat removal pump

inoperable for the low pressure coolant injection mode and 24 houre for the shutdown

cooling mode, while pursuing the calibration accuracy of test instrumentation as the

possible cause of the pump's failure to meet surveillance test acceptance criteria, la

addition, operations shift management directed and operators conducted a radioactive

waste discharge without first resolving operability concerns with lake blowdown flow

Indication. Finally, post-maintenance testing to ensure operability following replacement of

all four air start motors on an emergency diesel generator did not reflect the possible effect

of the maintenance on emergency diesel generator start time.

- Based on the results of this inspection, the NRC determined that three violations of NRC

requirernents occurred. These violations are cited in the enclosed Notice of Violation

(Notice) and the circumstances surrounding them are described in detailin the subject

inspection report. Two of the violations are of concern because they involve the non-

conservative approach to equipment operability discussed above. The third violation

involves a f ailure to apply appropriate radiological markings to tygon tubing installed in the

Jplant and indicates that plant personnellack knowledge of radiological administrative

controls.

You are required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response. The NRC will use your response, in part,

to determine whether further enforcement action is necessary to ensure compliance with

regulatory requirements.
