s ADVISORY COMMITTEE ON REACTOR SAFEGUARDS HUMAN FACTORS SUBCOMMITTEE MEETING MINUTES JUNE 3, 1997 ROCKVILLE, MARYLAND The Advisory Committee on Reactor Safeguards (ACRS) Subcommittee on Human Factors held a meeting on June 3, 1997, in Room T-2 B3, 11545 Rockville Pike, Rockville, Maryland, with representatives of the U.S.
Nuclear Regulatory Commission (NRC).
The purpose of this meeting was to hear presentations by and hold discussions with the NRC staff regarding the development a Human Performance and Reliability Implementation Plan. Mr. Noel Dudley was the cognizant ACRS staff engineer for this meeting.
The meeting was convened at 12:00 noon and adjourned at 3:00 p.m.

OPENIWG REMARKS BY THE SUBCOMMITTEE CHAIRMAN W
Dr. George Apostolakis, Chairman of the Human Factors Subcommittee, convened the meeting at 12:00 noon.
He stated that the purpose of the meeting was to hold discussions with the representatives of the NRC staff regarding the status of the Human Performance and Reliability Implementation Plan (Implementation Plan).
He noted that the Office of Nuclear Regulatory Research has been leading an agency-wide effort to develop the Implementation Plan, which will serve the NRC's future needs for confirmatory research and developmental projects.
Dr.
Apostolakis stated that the Subcommittee would gather information, analyze relevant issues and
- facts, and formulate proposed positions and
- actions, as appropriate, for deliberation by the full Committee.

June 3, 1997 NRC STAFF PRESENTATION Introduction - Mr. Wayne Hodges, RES Mr. Hodges explained that in response to comments by the ACRS and the Nuclear Safety Research Review Committee concerning the Human Performance Program Plan, the staff was developing an agency-wide plan to deal with human factor and human reliability issues.
He stated that the Human Performance and Reliability Implementation i
Plan (Implementation Plan) was a work-in-progress, which consisted j
of a catalogue of activities with a preface.
He noted that the staff was scheduled to complete a draft of the Implementation Plan by June 30, 1997.
Human Performance and Reliability Imolementation Plan - Dr. Julius Persensky, RES Dr. Persensky presented the vision statement, mission statement, strategy, and seven program areas of the Implementation Plan.
He outlined the organizational changes that had occurred to support preparation of the plan.
He explained how the ATHEANA (a technique for human event analysis) model of human reliability is a suitable framework for demonstrating the relationships among the NRC's human performance and human reliability activities.
Dr.
Persensky presented a sample table from the Implementation Plan that summarized program areas, objectives, activities, target schedule, and lead office.
He also provided an example of the planning sheets that the staff used to develop the summary table.

Persensky concluded the presentation by identifying the following three outstanding issues:
the adequacy of the regulatory framework to address human performance, e
the adequacy of human performance data to support research efforts, and
<the limited budgetary resources available to develop and implement the plan.
Discussion The Subcommittee Members and the staff discussed the structured planning process used to develop the Implementation Plan.
Dr.
Powers compared the framework of the Implementation Plan to the template from the IEEE standard on system engineering.
He questioned whether the staff considered the NRC budget as a constraint or a requirement.
Ms. Ann Ramey-Smith, RES, replied the budget was viewed as a constraint.

June 3, 1997 The Subcommittee Members and the staff discussed the vision statement.
Discussion items included rewriting the vision statement as a positive statement and the appropriateness of the word "disproportionally."
The staff agreed to review the mission statement in light of the Members' comments.
The Subcommittae Members and the staff discussed the following issues:

whether the staff perceives that the assumed risk worth attributed to human performance in probabilistic risk assessments was too high and should be reduced, whether achieving mission statements would be done primarily by rule changes, developing metrics, such as, uncertainty ranges or risk worth e
goals for mission statements, incorporating the concept of safety culture in the e
Implementation Plan, incorporating an understanding of organizational factors in e
the regulatory process, communicating the Implementation Plan to industry, and e
the perception that the staff does not participate in national or international conferences.
The Subcommittee Members and the staff discussions of the diagram of the ATHEANA model included the appropriateness of the term
" error-forcing factors," expansion of the diagram to include performance-shaping factors, and identifying interfaces among functions and betwee the functions and strategy elements.

Dr. Powers noted that the Implementation Plan established clear ownership and that a process model is being used to guide the design of the plan.
He noted that the strategies were not quantitative and that a metric was needed for assessing whether stated missions were completed.
Dr. Powers recognized that the establishment of interfaces between the different tiers of functions and program areas, and the verification of completeness and lack of redundancy among functions would be a difficult iterative process.
Dr. Apostolakis complemented the staff on inclusion of the ATHEANA model.
He recommended that the diagram of the ATHEANA model be adapted to include additional boxes to indicate the performance-h

He noted that the diagram could then be used to i
develop metrics for the performance-shaping factors.
Dr.
Apostolakis suggested that the staff define " safety culture" and differentiate between formal and informal cultures.
Dr. Seale stated that the' Implementing Plan had the appearance and contained the critical elements of a structured plan. He suggested that the staff work with the Institute of Nuclear Power Operations (INPO) to share human performance data.
Mr. Barton stated he was encouraged with the new direction taken by the staff.
FOLLOWUP ACTIONS The staff plans to provide the ACRS a copy of the Implementing Plan by June 30, 1997.
SUBCOMMITTEE RECOMMENDATION The Subcommittee recommended that the staff brief the Subcommittee on the results of the pilot test of the ATHEANA model at a nuclear power plant.
The pilot test is scheduled for July 1997.

comments on the plan at the October 1997 ACRS meeting.
