Page 2 of 4 The purpose of this modification is to: (1) )rovide for a within scope change under Section C of the basic contract, there)y increasing the level of effort for each contract year and (2) increase the ceiling amounts of the base year and option years as a result of the within scope change. Accordingly, the contract is hereby modified as follows:

Under Subsection C 5 entitled " SPECIFIC TASKS." incorporate the task of reviewing and coding inspection reports.

Therefore, paragraph c. is added to read as follov.3:

"c.

REVIEW AND CODE INSPECTION REPORTS Provide technical assistance in reviewing and coding approximately 1450 Inspection Reports per year using the HFIS Inspection Report coding instructions, and entering the human performance information extracted from the inspection reports into the HFIS database on a monthly basis."

Under Subsection B.3, " CONSIDERATION AND OBLIGATION--TASK ORDERS." as a result of adding an additional task under the contract, the level of effort for the base year and the option years is increased, thereby increasing the ceiling amounts for each contract year. The ceiling amount of the Base Year is increased by $87.132 from $22.622 to $109.754.

Option Year 1 is increased by $89.727 from $23.283 to $113.010. and Option Year 2 is increased by $92.401 from $23.964 to $116.365.

Therefore, paragraphs (a) through (c) under subsection B.3 are deleted in their entirety and substituted with the following in lieu thereof:

"B.3 CONSIDERATION AND OBLIGATION--TASK ORDERS (AUG 1989)

(a) The total estimated amount of this contract (ceiling) for the products / services, ordered, delivered, and accepted under the Base Year of the contract is $109.754.00.

The Contracting Officer may unilaterally increase this amount as necessary for orders to be placed with the contractor during the contract period provided such orders are within any maximum ordering limitation prescribed under this contract.

(b) The total estimated amount (ceiling) for the products / services ordered, delivered, and accepted under Option Year 1 is is $113.010.00. The Contracting Officer may unilaterally increase this amount as necessary for orders to be placed with the contractor during the contract period provided such orders are within any maximum ordering limitation prescribed under this contract.
