NRC ENGINEERING INSPECTION REPORT 50 254/97013(DRS);
50-265/97013(DRS) AND NOTICE OF VIOLATION

Dear Mr. Pearce:
On September 5,1997, the NRC completed an inspection at your Quad Cities Nuclear Power
Station. The purpose of the inspection was to review the post-modification testing of the
r'.tsidual heat removal service water (RHRSW) pumps' cut water and impeller modifications.
Additionally, your current inservice testing (IST) program related to the RHRSW pumps was
reviewed. The results of this inspection are presented in the enclosed report.
The inspectors noted that neither the post-modification testing nor the inservice testing ensured
the RHRSW pumps would perform within acceptable limits of applicable design documents.
Because of this, your staff was required to perform an operability evaluation of the RHRSW
pumps, in addition, your staff could not readily identify or explain the basis for the RHRSW
pump flow surveillance criteria which was in conflict with the Updated Final Safety Analysis
Report description of the RHRSW pumps.
The NRC identified one test control violation for falling to relocate the surveillance
requirements from technical specifications to your IST progism as part of your technical
specifications upgrade program (TSUP). This violation is a concern because the surveillance

requirement for the RHRSW pumps provides a basis for determining the operability of the

RHRSW system. The failure resulted in the operability status of tha RHRSW pumps not being

verified by testing since TSUP inception. Further, the inspectors noted that test control and
surveillance problems have been identified in recent NRC inspections (Inspection Reports 50-
254/265/96017,50-2v/265/96020 and 50 254/265/97011) and corrective actions taken to date
have not precluded these problems.
The violation is cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding it are described in detailin the enclosed report. Please note that you are required
to respond to this letter and should follow the instructions specified in the enclosed Notice when
preparing your response, in your response please address how you have assured yourself that
other technical specification surveillance requirements were appropriately relocated to your IST

program for other components that were affected by the TSUP. The NRC will use your
response, in part, to determine whether further enforcement action is necessary to ensure
compliance with regulatory requirements.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its
enclosures wiil be placed in the NRC Public Document Room.
