An NRC inspection team will arrive at your Sequoyah facility on May 4. 1998, to perform an inspection according to Temporary Instruction (TI) 2515/137, l

Inspection of Medium-Voltage and Low-Voltage Power Circuit Breakers, dated March 1998.

The team will comprise the following individuals:

Quality Assurance, Vendor Inspection and Maintenance Branch.

Frank Gee, NRR. Division of Reactor Controls and Human Factors.

Gregory Cwalina. NRR, Division of Reactor Controls and Human Factors.

Quality Assurance. Vendor Inspection and Maintenance Branch.

In addition. Julio Crespo who is employed by the Spanish equivalent of the USNRC will observe the inspection.

inspection room at the start of the inspection.

Information to allow selection of a sample of purchase orders (technical portion) for parts or services related to switchgear for review.

Both safety-related and commercial grade dedication will be selected. After selection early in the inspection, the purchase order records will be requested.

Description of vendor interface program, including relevant procedures.

A copy of your reply to Generic Letter 83-28. Item 2.2. Part 2: and a copy of your response to Generic Letter 90-03 and its Supplement 1.

Maintenance procedures for switchgear.

If recently substantially revised a copy of previous revision is also requested.

4.

Instruction manuals for switchgear.

5.

One-line diagrams depicting switchgear.

6.

Typical elementary diagrams for breaker control.

7.

Copy of last two preventive / predictive maintenance work request data sheets for the following breakers:

Emergency diesel generator 1A output breaker 6.9 kV shutdown board 2A normal incoming supply breaker 6.9 kV shutdown board 1B alternate incoming supply breaker Emergency raw cooling water 2B feeder breaker 480 V shutdown board 1Al-A containment air return fan 1A-A feeder breaker 480 V shutdown board 182-B normal incoming supply breaker 480 V shutdown board 2Al-A component cooling system pump 2A-A feeder breaker 480 V shutdown board 2B1-B normal feeder to reactor vent board 2B-B 8.

Corrective maintenance history for medium-voltage and low-voltage breakers since beginning 1995 or list of all breaker failures for same period.

Copies of any root cause evaluations for circuit breaker failures or problems.

Problem reports written for switchgear problems and issues covering a period of time encompassing two refueling outages.

We also request that the following documents be available for our review at their normal storage location.

1.

Copies of letters from switchgear manufactures on the subject of potential problems and site specific evaluation for same.

Copies of NRC communications on the subject of problems with switchgear and site specific evaluation for same.
